Or, if you prefer, print off a patient consent form to complete in your own time and either post it or bring it in person along with your repeat slip to the ARC Pharmacy branch of your choice. With your consent, a branch of ARC Pharmacy will able to re-order your repeat prescription from any of the surgeries in Selby for you. You can give your consent by telephone, fax or email, or by leaving your repeat slip which will be securely stored at your selected branch of ARC Pharmacy.
